Committee Position Nominations
Nominate yourself or another member for a committee position using this form. Forms must be physically signed and received by the secretary no later than Monday the 20th July 2026. The last club meeting before the AGM is on Monday the 13th July 2026.

Proxy Voting
Nominate another member to act as your proxy if you are unable to attend the AGM but would like your vote counted using this form.
Proxy forms must be signed and dated and presented to the Chair Person running the AGM prior to the meeting commencing.
